We are seeking a compassionate and organized Registered Nurse (RN) to support an Enhanced Care Management (ECM) program within a community health setting. This role focuses on care coordination for high-risk patients, helping them manage chronic conditions, navigate the healthcare system, and reduce avoidable hospital or emergency room visits.
This is a non-bedside nursing role. The ECM RN works collaboratively with providers, case managers, and community partners to deliver coordinated, patient-centered care.

Position Summary
The ECM Registered Nurse conducts assessments, supports care planning, and coordinates services for high-risk patients. This role emphasizes population health, patient education, and continuity of care across healthcare and community-based services.

Key Responsibilities
Conduct nursing assessments for high-risk patients
Review medical history, medications, and ongoing care needs
Assist in the development and ongoing updates of individualized care plans
Support patients following hospital or ER discharges
Perform medication reconciliation and ensure follow-up appointments are scheduled
Collaborate with providers, behavioral health teams, and case managers
Educate patients and families on disease management and medications
Advocate for patient needs within the healthcare system
Document all care coordination activities accurately in the electronic health record
Participate in care team meetings and quality improvement initiatives
Travel locally as needed

Minimum Qualifications
Active California Registered Nurse (RN) license
Minimum of 2 years of clinical nursing experience
Strong communication, organizational, and documentation skills
Compassionate, patient-centered approach
Ability to work independently and as part of a collaborative team
Valid Driver’s License
Current BLS certification

Preferred Qualifications
Experience with care management, case management, or high-risk patient populations
Familiarity with Medi-Cal, CalAIM, or community health settings
Bilingual skills (Spanish, Punjabi, or Portuguese)

Schedule & Work Environment
Part-time, approximately 28 hours per week
Must be available Monday–Saturday between 8:00 AM – 5:00 PM, as scheduled
On-site role with some local travel
Non-exempt, temporary assignment (12 months)
